Benchmark Human Services-GA is seeking an Intensive Support Coordinator to help individuals with disabilities live more independently and participate in their community. You will provide resources, plan care, and advocate for services under HCBS guidelines.

Qualifications

• Bachelor degree in human services, social work, sociology or psychology.
• Must be able to work collaboratively while meeting individual performance goals.
• Must be able to verbally and non‑verbally communicate with a broad audience of stakeholders.

Responsibilities

• Follow all policies and procedures set forth by the company and HCBS rules.
• Perform evaluations and assessments to meet the needs of individuals served.
• Assist individuals served with applying for financial assistance, planning, healthcare and in-home supports.
• Advocate for services to support the individual’s success.
• Maintain all case records and care path plans.
• Coordinate meetings with individuals, families and guardians.
