Job summary

Benchmark Human Services in Georgia seeks an Intensive Support Coordinator to help individuals with intellectual and developmental disabilities live more independently and participate in their communities.

You will conduct evaluations, assist with financial aid, develop Care Path Plans, coordinate teams, and advocate for services while maintaining records and tracking outcomes. Travel up to 75% is required; training and growth opportunities are provided in a mission-driven environment.

Qualifications

• Bachelor degree in human services, social work, sociology, psychology.
• Valid driver’s license and auto insurance.
• Minimum of 2 years in a similar position.
• Willing to travel up to 75%.

Responsibilities

• Follow all policies and procedures set forth by the company and state HCBS rules.
• Perform evaluations and assessments to meet the needs of individuals served.
• Assist individuals served with applying for financial assistance, residential planning, vocational, recreational and educational services.
• Advocate for services that support the individual’s success.
• Maintain all case records and prepare/update Care Path Plans.
• Coordinate meetings with individuals, families, guardians and interdisciplinary team members.
• Establish positive relationships with individuals, families, guardians, state officials and team members.
• Record work and billing time per policy; track data points.
