Intensive Support Coordinator - Chatham and Surrounding Counties

CareStar, Inc. is currently seeking an Intensive Support Coordinator in the Clinical department to identify, coordinate, and oversee the delivery of services to enhance the health, safety, and general well-being of participants to assist the participants' goals toward maximum independence.

THE POSITION

CareStar is looking for motivated individuals who:

  1. Bachelor’s degree in human service Field such as Social Work, Psychology, Sociology, Counseling, Gerontology, or Nursing (or closely related field).
  2. Two years of direct service experience serving persons with DD/IDD or related disabilities.
  3. Two years’ experience in healthcare, habilitative/rehabilitative, residential, behavioral health setting (ICF/IDD), psychiatric inpatient hospital/facility, specialty clinic, special education department, or similar setting.
  4. Experience in completing assessments, service plan development, and monitoring.
  5. Effective organizational and communication skills.
  6. Experience providing care management/coordination preferred.
  7. Willingness and ability to work a flexible schedule to meet the needs of families including reliable transportation, valid driver's license, and car insurance as required by state law, and the ability to travel.
  8. Adheres to the CareStar Rule in performance of job responsibilities.
  9. Understands and complies with CareStar Policies and Procedures.
  10. Maintains confidentiality related to patient information. Any unlawful disclosures of confidential information made outside the proper course of duty will be treated as a serious disciplinary offense.
  11. Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

This position is open in Chatham County and is responsible for covering the surrounding counties.
