CareStar, Inc. is seeking an Intensive Support Coordinator in the Clinical department to identify, coordinate, and oversee the delivery of services that enhance the health, safety, and well-being of participants, helping them achieve maximum independence. This position covers Dougherty County and surrounding areas.

The Position

We are looking for motivated individuals who have:

• A bachelor’s degree in human services such as Social Work, Psychology, Sociology, Counseling, Gerontology, Nursing, or a related field.
• At least two years of direct service experience serving persons with DD/IDD or related disabilities.
• Experience in healthcare, habilitative/rehabilitative, residential, behavioral health settings, psychiatric inpatient facilities, specialty clinics, or similar environments.
• Skills in assessments, service plan development, and monitoring.
• Effective organizational and communication skills.
• Experience in care management/coordination is preferred.
• Ability and willingness to work flexible hours, reliable transportation, valid driver's license, and ability to travel.
• Adherence to CareStar's policies, confidentiality, and acceptable use policies.

This role is based in Dougherty County, covering surrounding areas.
