Intensive Support Coordinator - Colquitt and Surrounding Counties

We are seeking an Intensive Support Coordinator in the Clinical department to identify, coordinate, and oversee the delivery of services that enhance the health, safety, and well-being of participants, helping them achieve maximum independence.

THE POSITION

CareStar is looking for motivated individuals who:

• Bachelor’s degree in human services fields such as Social Work, Psychology, Sociology, Counseling, Gerontology, Nursing, or a closely related field.
• Have two years of direct service experience serving persons with DD/IDD or related disabilities.
• Possess two years’ experience in healthcare, habilitative/rehabilitative, residential, behavioral health settings (e.g., ICF/IDD), psychiatric inpatient facilities, specialty clinics, or similar environments.
• Have experience in completing assessments, developing, and monitoring service plans.
• Demonstrate effective organizational and communication skills.
• Experience in care management/coordination is preferred.
• Are willing and able to work a flexible schedule to meet family needs, possess reliable transportation, a valid driver's license, and insurance, and are able to travel.
• Adhere to the CareStar Rule and comply with company policies and procedures.
• Maintain confidentiality regarding patient information and understand the importance of lawful disclosures.
• Follow the Acceptable Use Policy for all CareStar information systems.

This position is based in Colquitt County and covers surrounding counties.
