• Conduct psycho-social assessments and develop Person‑Centered Recovery Plans.
• Assist clients in accessing community resources and provide medication education.
• Document services in electronic health records and collaborate with a multidisciplinary team.
• Deliver intensive support services through ACT Team with a focus on crisis intervention when needed.
