Qualifications

• Bachelor's degree or higher in aHuman Services field is required or 24 college credits in Human Services if degree is not in Human Services.
• Experience in behavioral healthcare, intensive community treatment, and substance use treatment.
• Valid Texas Driver's License and driving status maintained.

Responsibilities

• Provide intensive, person-centered case management and rehabilitative skills training to a small caseload of adults with mental health and substance use treatment needs.
• Meet individuals in homes, community settings, hospitals, crisis units, and clinics to achieve recovery outcomes.
• Coordinate care with stakeholders and community providers, using an electronic medical record system.
• Participate in team meetings, training opportunities, and crisis planning with a focus on independence.
• Work with supervision to balance rapid crisis response, advocacy, and recovery planning.

Education

Bachelor's degree in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education/intervention
