The Outpatient Navigator, under supervision, facilitates in-patient transitions into Oceans Outpatient Program upon patient discharge and thereafter in accordance with Joint Commission, Federal and State Regulations, the Oceans' Mission and Performance Improvement

Responsibilities

• Facilitates inpatient transitions into Oceans Outpatient Program upon discharge.
• Educates inpatient groups about IOP services and benefits of the program.
• Manages admit process for patients stepping down into IOP including documentation.
• Coordinates with case management for discharge planning and involves patient's support system.
