Job summary

Daymark Recovery Services is seeking an IIH Team Member to coordinate initial and ongoing assessments for youth, convene the child and family team, and develop the Person Centered Plan with input from providers and families.

You will link to clinical and community resources, arrange for evaluations, coordinate behavioral health services, and monitor progress to inform PCP outcomes. A degree in human services and relevant experience are required.

Qualifications

• Minimum of an Associates or Bachelor’s degree in human services or related field.
• At least 2 years post-degree experience with children/adolescents with MH/SU diagnoses.

Responsibilities

• Coordinate and oversee initial and ongoing assessment activities.
• Convene the child and family team for person-centered planning.
• Develop and implement the Person Centered Plan with PCP input.
• Collaborate with medical and non-medical providers to link services.

Pay Scale:$23.07/hr.- $24.04/hr.

Summary:

Coordinates and oversees assessment activities, convenes Child and Family Team, consults with community and professional resources, and links to appropriate services. Also provides and coordinates behavioral health services and other interventions, and participates in the PCP planning process. Provides crisis intervention to youth recipients; monitors and documents the status of client progress.

The responsibilities of the IIH Team Member include, but are not limited to, the following:

• Coordinating and overseeing the initial and ongoing assessment activities
• Convening the child and family team for person-centered planning
• Completing the initial development and ongoing revision of the Person Centered Plan and ensuring its implementation
• Consulting with identified medical and non-medical providers, engaging community and natural supports, and including their input in the PCP process
• Ensuring linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations
• Providing and coordinating behavioral health services and other interventions for the youth or other family members with other licensed professionals and Child and Family Team Members
• Monitoring and documenting the status of the recipients progress and the effectiveness of the strategies and interventions outlined in the PCP

Under the direction of the team leader:

• Participating in the PCP process
• Assisting with implementing a home based behavioral support plan with the youth and his or her caregivers as indicated in the PCP
• Providing psychoeducation as indicated in the PCP
• Assisting the team leader in monitoring and evaluating the effectiveness of interventions, as evidences by symptom reduction and progress towards goals identified in the PCP
• Assisting with Crisis Interventions
• Assisting the team leader in consulting with identified providers, engaging community and natural supports, and including their input in the person-centered planning process

Education and/or Experience:

This position requires a minimum of an Associates or Bachelor’s degree in human services or a related field and 2 years post degree experience working with children/adolescents who have MH/SU diagnoses.
