Responsibilities

• Write and implement plans of care approved by the consulting clinician.
• Maintain frequent communication with Family Service Workers to ensure compliance with the family’s case plan.
• Provide services to families in home and community settings.
• Assist families in locating resources for housing, transportation, counseling, and nutrition.
• Coordinate services with schools and other providers and participate in meetings and trainings.
• Provide in-home family support and crisis planning when needed.
• Testify in court concerning services when subpoenaed.
