Job summary

Easterseals NH & VT invites an Intensive Family Based Services Coordinator to join our team. The role focuses on writing care plans, coordinating with clinicians and family service workers, and delivering in-home supports to families in need.

You will facilitate planning, monitor progress, and help connect families to housing, transportation, treatment, and nutrition services. Strong writing and communication skills are essential, with field transportation responsibilities included.

Qualifications

• Bachelor’s Degree in a related field.
• Two (2) years of experience preferred.
• Excellent writing skills necessary.
• Valid driver’s license and reliable transportation.

Responsibilities

• Write and implement plans of care approved by the consulting clinician.
• Write contact notes, monthly reports and discharge summaries.
• Maintain frequent communications with Family Service Workers to ensure compliance with the family’s case plan.
• Provide services to families in their home and community settings.
• Assist families in finding resources for housing, transportation, treatment, counseling and nutrition.
• Support caregivers to meet children’s needs and build confidence and skills in a safe environment.
• Participate in meetings, trainings and events as assigned.
• Provide transportation for clients when required.
• Testify in court when subpoenaed.
• Ensure timely Family Assessments per IFBS policies.
• Create crisis plans and respond to crisis calls as needed.
• In-home family support aligned with family schedules.
• Implement short-term (13 week) interventions to preventDCF involvement or reunify families.
