Easterseals VT is seeking an Intensive Family Based Services Coordinator in Burlington, VT. You will write and implement plans of care, maintain timely assessments, and coordinate services with schools and clinicians to support families in home and community settings.

The role requires a Bachelor's degree, strong writing skills, and a valid driver’s license. You’ll work 40 hours per week, with opportunities for flexible scheduling and a comprehensive benefits package including

Qualifications

• Bachelor’s degree in a related field.
• Two years of experience preferred.
• Excellent writing skills and valid driver’s license.

Responsabilités

• Write and implement plans of care approved by the consulting agency clinician.
• Write contact notes, monthly reports, and discharge summaries.
• Maintain frequent communications with Family Service Workers to ensure compliance with the family’s case plan.
• Provide services to families in home and community settings.
• Assist the family in finding resources for housing, transportation, and treatment services.
• Help caregivers to recognize children’s needs and implement strategies to meet them safely.
• Participate in meetings, conferences, trainings and events.
• Provide transportation for clients when required.
• Testify concerning services in court when subpoenaed.
• Coordinate services with Schools, Early Intervention services, clinicians and other providers.
