Job summary

Scharpca seeks an Intensive Case Manager for Permanent Housing in Los Angeles County to provide targeted services for clients transitioning from homelessness to permanent housing. You will serve as the central point of contact, coordinating with DMH, DHS and health partners to stabilize clients with chronic illness or disabling conditions.

The role involves conducting assessments, developing tailored service plans, and assisting with housing location, budgeting, and crisis support.

Qualifications

• BA degree preferred and relevant experience in social services.
• Excellent written and verbal communication required.
• Proficient with Windows OS and electronic health record systems.

Responsibilities

• Receive referrals, assess eligibility and enrollment to the program.
• Collaborate with DMH, DHS and other programs to meet client needs.
• Assist clients with access to temporary housing until permanent placement is secured.
• Develop and implement an individualized case management services plan.
• Monitor and update client case plans at least every 3 months.
• Provide transportation as needed to appointments and housing visits.
• Assist clients in locating permanent housing and negotiating rental agreements.
• Establish relationships with landlords and housing agencies.
• Support budgeting and money management for clients.
