Job summary

Mental Health America of Los Angeles (MHALA) is seeking a Personal Service Coordinator/Intensive Case Manager to provide high-quality ICMS to clients eligible for permanent housing through the HFH program. You will manage a caseload of up to 25 high-acuity members and coordinate move-in and access to housing services.

Responsibilities include intake, enrollment, assisting with income/benefits, employment, transportation, and documenting in EHR; participate in crisis prevention rotations;

Qualifications

• High school diploma or GED required; 2 years’ psychosocial rehab-related experience preferred.
• Proficient with Word, Excel and Outlook; able to learn HMIS and EHR systems.
• Experience with clients who have mental illness, chronic health issues, and substance use disorders.
• Ability to work in a team-oriented environment and flexible scheduling.

Responsibilities

• Provide ICMS to clients eligible for permanent housing through HFH with a caseload up to 25 high-acuity members.
• Perform intake/enrollment, assist with housing move-in, applications, and paperwork.
• Help clients obtain income/benefits, employment, transportation, basic needs and life skills.
• Educate clients on tenant rights, eviction prevention, and housing stability with HFH partners.
• Document in EHR; 24-hour turnaround; participate in On-call crisis prevention rotation.
• Engage in professional development activities to maintain skills.
