Job summary

Mental Health America of Los Angeles (MHALA) is seeking a Personal Service Coordinator/Intensive Case Manager to deliver high-quality ICMS to clients eligible for permanent housing through the HFH program. You will provide a client-centered approach and coordinate with DHS, HFH partners, and housing providers to support housing stability.

The role involves case management for up to 25 high-acuity members, move-in coordination, documentation in EHR, and development of individualized service

Qualifications

• High school diploma or GED with 2 years’ psychosocial rehab experience.
• Ability to work in a team-oriented, flexible environment.
• Proficiency with basic computer applications (MS Word, Excel, Outlook).

Responsibilities

• Provide Intensive Case Management Services for housing program clients.
• Coordinate move-in activities and enrollment into ICMS for eligible clients.
• Maintain documentation in EHR and ensure timely turnaround.
• Develop individualized service plans and connect clients to health, mental health, and benefits.
