Job summary

Blue Kite Wellness is seeking an Intensive Home-Based Clinician II to deliver clinical services to children, youth, and families in home and community settings in Pontiac, IL, with telehealth when permitted.

The role provides assessment-informed, MAP-based interventions, collaborates with families, Care Coordinators, and schools, and requires QMHP/LPHA eligibility and a Master’s degree. The position is field-based with travel and a salary range of $55,000–$65,000 plus a $2,000 sign-on bonus.

Qualifications

• Meets current Illinois requirements for a QMHP or LPHA.
• Master’s degree in social work, counseling, psychology, or related field.
• At least two years of behavioral health experience with children and families.
• Strong assessment, engagement, treatment planning, and crisis-prevention skills.
• Independently work in home and community settings with a multidisciplinary team.

Responsibilities

• Provide intensive home-based clinical services per service plans and Illinois Pathways requirements.
• Develop and adjust individualized clinical interventions using IM+CANS and assessments.
• Deliver individual, family, and caregiver therapy to improve symptoms and family functioning.
• Strengthen caregiver capacity, family communication, and coping skills.
• Provide clinical direction to IHBS staff and review progress.
• Collaborate with caregivers, schools, and other supports within the Child and Family Team.
• Support crisis prevention, safety planning, and post-crisis follow-up.
• Document accurately per Blue Kite Wellness and payer requirements.
