Job summary

The Villages of Indiana is seeking a Case Manager for Intensive Foster Care to lead intake, assessments, and treatment planning. You will coordinate services with the care team and serve as the main liaison with the DCS FCM, attending CFTM meetings as needed.

You will work closely with foster families to provide support, trainings, respite options, and activity coordination, while guiding behavior management and skill-building for both families and children.

Qualifications

• Bachelor’s degree from an accredited four-year college.
• If BSW, then LBSW is needed.
• Five (5) years of supervised, paid casework experience.
• One (1) year of graduate training in a recognized school of social work may substitute for the required five years of supervised, paid case work experience.

Responsibilities

• Handle intake and implement Assessments to develop Treatment Plan and Family Support Plan.
• Coordinate services with team members to address needs.
• Be the main contact with the DCS FCM and attend CFTM meetings as needed.
• Support foster parents with trainings, groups, respite, activities, and behavior management guidance.
