Job summary

The Villages of Indiana is hiring a Case Manager – Intensive Foster Care to support children and families through coordinated treatment planning and service delivery. The role requires collaboration with team members and DCS FCM, plus participation in CFTM meetings as needed.

Responsibilities include intake, assessments, family support planning, and working with foster parents to provide trainings, respite options, activities, behavior support, and skill-building for families and children.

Qualifications

• Bachelor’s degree from an accredited four-year college.
• If BSW, LBSW is needed.
• Five years of supervised, paid casework experience; two in an approved child placing agency.
• One year of graduate training in social work, may substitute five years of experience.

Responsibilities

• Handle intake and assessments to develop Treatment Plan and Family Support Plan.
• Coordinate services with all team members.
• Be main contact with DCS FCM and attend CFTM meetings as needed.
• Support foster parents with trainings, respite, activities, and skill-building.
