Job summary

Youth Advocate Programs, Inc. is seeking an Intensive Family Coordinator to deliver professional case management and direct advocacy for a caseload of families.

The role focuses on completing family assessments and developing individualized service plans to support youth and families. The position requires coordinating home studies, recruiting foster parents, and providing 24/7 crisis support, with occasional travel and staffing evenings and weekends.

Qualifications

  • High school diploma or GED required.
  • 1+ year experience in human services or community work.
  • Experience in home and community-based services.
  • Proficient in MS Office; knowledge of EHR a plus.

Responsibilities

  • Complete family assessments and create individualized service plans.
  • Complete home studies for the referring authority and recruit foster parents.
  • Provide case management including intakes, safety planning, ISP development, and 24/7 crisis support.
  • Maintain case records and coordinate with teams; travel as needed and work nights/weekends.

Job description

Summary of the Position: The Intensive Family Coordinator will provide a professional level of case management services as well as direct advocacy services to a caseload of families.

  • The Intensive Family Coordinator will be responsible for completing family assessments and creating individualized service plans.
  • IntensiveFamily Coordinator will be responsible for completing family assessments, recruiting foster parents, completing home studies for the referring authority, locating family resources, and creating individualized service plans.
  • Case management services including intakes, safety planning, team meetings, ISP development 24/7 support, case contacts, and case record documentation in keeping with state regulations to build a network of support for each youth and family referred.
  • Travel will be necessary as well as willingness to work nights and weekends and provide 24/7 crisis support to the caseload.

Qualifications/Requirements:

  • Minimum High School Diploma or GED Equivalent.
  • Experience with community work and at least one (1) year experience working in human services fields.
  • Experience in the provision of home and community-based services.
  • Proficiency in MS Office (Word, Excel, Outlook, etc.)
  • Knowledge of Electronic Health Record (EHR) systems is a plus.
  • Strong written and verbal communication skills.
  • Exceptional organization skills and attention to detail.
  • Reliable transportation, valid driver’s license, and current auto insurance coverage is required.
  • Bilingual/Spanish speaking is a plus.
