Youth Advocate Programs, Inc. is seeking a Case Manager for the Orange County Improving Families Program.

The successful candidate will organize and implement direct services for assigned youth and families, using a wrap-around, strength-based approach within our community-focused initiatives. The role demands experience with at-risk youth and families, strong communication, and the ability to work effectively in neighborhood-based contexts.

Qualifications

• Bachelor's degree is required.
• Experience working with at-risk youth and families.
• Proficient in Microsoft Office Suite; EHR experience a plus.
• Strong communication and interpersonal skills.
• Position requires reliable transportation, valid driver’s license, and auto insurance.
• Bilingual (Spanish speaking) is a plus.

Job description

Summary of Position: Case Manager working for the Orange County Improving Families Program. The successful applicant will possess stellar credentials in working successfully with youth, families and communities to safely manage troubled families in their homes with intensive case management and wrap-around support.

• Responsibility of Case Manager is to initiate, organize, plan, develop, and ensure implementation of direct services to assigned participants and families.
• All service plans will be based on a strength-based approach using the wrap around model.
• Successful applicants will be well versed in child welfare and neighborhood-based approaches to aiding youth and families in crisis

• Proficient in Microsoft Office Suite. Experience using an electronic health record system is a plus.
