Job summary

Youth Advocate Programs, Inc. is seeking an Intensive Family Coordinator to provide professional case management and direct advocacy to a caseload of families.

The role involves conducting assessments, planning services, and coordinating resources in line with state regulations. You will handle intakes, safety planning, team meetings, ISP development, and ongoing case documentation, with potential evening or weekend hours.

Qualifications

  • High School Diploma or GED is required.
  • Experience in community work and in the human services field.
  • Experience in home and community based services.
  • Proficient in Microsoft Office Suite; EHR system experience a plus.
  • Strong communication and organizational skills.
  • Must have reliable transportation, a valid driver’s license, and current auto insurance.
  • Bilingual Spanish speaking is a plus.
  • Evenings and weekends may be required.

Summary of the Position

Intensive Family Coordinator provides a professional level of case management services as well as direct advocacy services to a caseload of families.

Responsibilities
  • Complete family assessments, recruit foster parents, complete home studies for the referring authority, locate family resources, and create individualized service plans.
  • Provide case management services including intakes, safety planning, team meetings, ISP development, 24/7 support, case contacts, and case record documentation in keeping with state regulations.
Qualifications / Requirements
  • High School Diploma or GED is required.
  • Experience with community work and at least one year experience working in the human services field.
  • Experience in the provision of home and community based services.
  • Proficient in Microsoft Office Suite and computer skills. Experience using an electronic health record (EHR) system is a plus.
  • Strong communication and organizational skills.
  • Must have reliable transportation, a valid driver's license, and current auto insurance coverage.
  • Bilingual (Spanish Speaking) is a plus.
  • May require evenings and weekends.
