Job summary

Highland Rivers Behavioral Health is seeking an Intensive Case Manager to join its community-based team. The role develops Individual Recovery Plans (IRP) for adults with serious mental illness and coordinates medical, behavioral, social, and housing services to prevent out-of-home placement.

Responsibilities include visiting individuals in their homes, providing transportation, linking to CORE services, and maintaining documentation and caseloads according to policy.

Qualifications

• Bachelor’s degree required.
• 21 years of age or over.
• At least 1 year of professional experience in behavioral health or substance use disorder services preferred.
• Background check, official transcripts, valid driver’s license/ID, drug screen, TB test, and eligible 7-year Motor Vehicle Report required.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide case management services in community settings.
• Develop and implement Individual Recovery Plans (IRP) addressing medical, behavioral, social, educational, vocational, housing, and financial needs.
• Visiting clients in their homes and providing transportation.
• Coordinate with CORE services providers and other resources to meet IRP goals.
• Maintain documentation and ensure caseloads meet program policies and HIPAA requirements.
• Attend team meetings and participate in program supervision.
