Job summary

Highland Rivers Behavioral Health is seeking a dedicated Case Manager to support the ROC Resiliency Clubhouse, helping youth with mental health issues build self-esteem, resiliency and life skills in a safe environment. The Case Manager will work from Mondays through Thursdays from 10:30am to 7:30pm and Fridays from 8:00am to 4:00pm.

You will coordinate rehabilitation services with individuals and service providers, develop and monitor the individual service plan, and maintain required paperwork

Responsibilities

• Assist individuals with service needs, developing strategies and supportive social support networks and coordinating rehabilitation services as specified in the individual's recovery plan.
• Partner with the individual and service providers in the development of the individual service plan.
• Provide crisis support and intervention as needed.
• Provide assessment, monitoring, and assistance to individuals.
• Establish professional relationships with individuals, families, and coworkers
• Complete all necessary paperwork and ensure eligibility criteria are met or referrals are made
• Manage caseloads, ensure services are authorized, and keep charts up to date
• Perform other duties as assigned

Duties and Responsibilities

• Assist individuals with service needs, developing strategies and supportive, social support networks and coordinating rehabilitation services as specified in the individual's individual recovery plan.
• Partner with the individual and service providers in the development of the individual service plan.
• Provides assistance and support in crisis situations.
• Provides assessment, monitoring, and assistance to individuals.
• Establishes professional and ethical relationships with individuals, families, and coworkers
• Provides direct service to individuals; completes all necessary paperwork according to policy; ensures individual meet eligibility criteria or refers to appropriate provider in the community.
• Manages a caseload by ensuring all services are authorized, chart paperwork is complete and current, and consumer charts are closed according to Agency guidelines.
• Other duties as assigned.
