Job summary

Highland Rivers Behavioral Health seeks an Intensive Case Manager to join the ICM Team in delivering community-based services in Whitfield, Murray, and Gordon Counties.

The role focuses on developing Individual Recovery Plans (IRPs), coordinating medical, behavioral, social, housing, and other supports, and linking clients to community resources to prevent unnecessary hospitalizations and improve recovery outcomes.

Qualifications

• This is a community-based case management role serving adults with mental illness and related conditions.
• Ability to develop and monitor Individual Recovery Plans (IRP) with client-centered goals.

Responsibilities

• Community based position.
• Provide transportation for individuals.
• Must visit individuals' homes.
• Assist individuals with service needs and coordinate rehabilitation services as specified in the IRP.
• Partner with the individual and CORE services provider in the IRP development.
• Link and advocate with other agencies to secure resources.
• Provide crisis intervention and ongoing assessment and support.
• Complete paperwork and ensure eligibility and charting compliance.
• Maintain caseload and document communications and service attempts.
• Attend treatment team meetings and support program supervision.
• Maintain client confidentiality per agency policies and HIPPA.
