Job summary

Directions for Living is seeking an Intensive Case Manager for the TW 2.0 Program in the Outpatient Services Division. You will provide wraparound services for children who have experienced trauma, in office or community settings, coordinating family-centered care.

The role requires experience in human services with children, a valid driver’s license, and progress toward targeted certifications within specified timelines, using evidence-based practices to build resilience.

Qualifications

• Two years' experience in human services or child welfare with a Bachelor's degree.
• Must maintain reliable vehicle and safe driving record.
• Must obtain targeted case management certification and Medicaid billing number.
• Must become Provisional Certified Behavioral Health Case Manager within 90 days and fully certified within one year.
• Must become Wraparound Facilitator in High Fidelity Wraparound within 3 months.

Responsibilities

• Provide Intensive Case Management services in the TW 2.0 program.
• Deliver services in office or community settings.
• Utilize High Fidelity Wraparound model and family-centered, evidence-based practices.
• Assess and coordinate services for children with trauma, loss, or behavioral needs.
• Assist with problem solving, behavior strategies, and anger management.
