Job summary

Directions for Living is seeking a professional Intensive Case Manager to deliver wraparound, family-centered services as part of the Treatment Works 1.0 (COP) program in the Pinellas County School system. This field role focuses on engaging youth at risk of violence and coordinating mental health and support services to keep them safe in school and at home.

The candidate will operate within a team-based approach, ensuring timely access to treatment, and will work towards long-term behavior

Qualifications

• Five years of experience in human services or child welfare with a Master’s degree in social work or a related area.
• Certified as a Child Welfare Professional, preferred.
• Must have a reliable vehicle with a safe driving record and maintain appropriate auto insurance; annual vehicle safety inspection if transporting clients.
• Must be eligible for targeted case management certification and Medicaid billing provider number.
• Must become certified as a Wraparound Facilitator in high fidelity Wraparound within 3 months of employment.

Responsibilities

• Field position providing Intensive Case Management services in the TW 1.0 program under the Outpatient Services Division.
• Responsible for achieving an annual client-facing care hours target of 1410 hours and ensuring timely access to services.
