Job summary

The Methodist Home of Kentucky seeks an Intensive Case Manager (ICM) for its Home Study and Post Release Services program in Kentucky. The role delivers comprehensive case management for families, coordinates care, and works flexible hours including weekends.

Requirements include a Master’s degree in social work or related field with clinical experience, or a BA with 5 years of clinical experience; bilingual in English and Spanish; knowledge of immigrant services; willingness to travel; and

Qualifications

• Master’s degree in a relevant behavioral science with clinical experience or a BA with 5 years of clinical experience.
• Fluent in English and Spanish (oral and written).
• Knowledge of local community social services and immigrant populations.
• Licensed or licensure-eligible (CSW/LCSW, LPCA/LPCC, AMFT/LMFT) preferred.

Responsibilities

• Deliver comprehensive mental health services and psychoeducational support.
• Collaborate with families and case management to set goals and plans.
• Conduct weekly in-home visits for at least 30 days and monthly contact for 6+ months.
• Provide in-person support during stressful or traumatic experiences.
• Document all contacts and assessments per standards.
• Develop trauma-informed clinical interventions for youth adjusting to life in the U.S.
• Perform screenings and provide referrals as needed.
• Travel frequently, including overnight and out-of-area travel.
