Job summary

  • Licensed or licensure-eligible (CSW/LCSW, LPCA/LPCC, AMFT/LMFT) preferred.

Responsibilities

  • Deliver comprehensive mental health services and psychoeducational support.
  • Collaborate with families and case management to set goals and plans.
  • Conduct weekly in-home visits for at least 30 days and monthly contact for 6+ months.
  • Provide in-person support during stressful or traumatic experiences.
  • Document all contacts and assessments per standards.
  • Develop trauma-informed clinical interventions for youth adjusting to life in the U.S.
  • Perform screenings and provide referrals as needed.
  • Travel frequently, including overnight and out-of-area travel.

About Home Study and Post Release Services

KyUMH works in collaboration with the Office of Refugee Resettlement (ORR) to support children who are placed with caregivers across Kentucky and potentially contiguous states. The Home‑Study Post Release Services Program (HS/PRS) aims to assess the safety and suitability of a sponsor before an unaccompanied child is released into a sponsor’s care, and to provide case management support to ensure the continued safety and stability of children in sponsor homes while linking them to community resources.

The PRS Intensive Case Manager (ICM) provides intensive case management services for families needing additional support. The primary focus is stabilizing the family unit through tailored interventions and connecting the released child and sponsor family to community‑based services for long‑term care. ICMs deliver Level 3 Post Release Services and may also provide Level 2 and Level 1 services. This role requires flexibility to work outside standard business hours, including evenings and weekends, as the HS/PRS program operates on a 24/7 schedule. Team members coordinate schedules to meet client needs, with approval from the Program Director or PRS Case Management Supervisor.

Education and Experience Requirements
  • Master’s degree in social work, psychology, sociology, or a relevant behavioral science with direct clinical experience required, OR Bachelor’s degree with a minimum of 5 years of clinical experience.
  • May be licensed or eligible for licensure (i.e., CSW/LCSW, LPCA/LPCC, AMFT/LMFT).
  • Highly fluent in English and Spanish (oral and written).
  • Knowledge of local community social services.
  • Specialized experience working with immigrant populations.
  • Knowledge of family preservation, kinship care, and child welfare.

Essential Functions
  • Deliver comprehensive mental health services, including psychoeducational support for recently migrated youth.
  • Collaborate with youth, families, and case management team to develop short‑term goals and treatment plans.
  • Conduct weekly in‑home visits for at least 30 days and maintain monthly contact for a minimum of 6 months.
  • Provide in‑person support during stressful or traumatic experiences (e.g., medical appointments, hospital visits).
  • Document all client contacts, interventions, and assessments in compliance with KYUMH and ORR standards.
  • Develop culturally competent clinical interventions to address trauma and assist youth in adjusting to life in the U.S.
  • Conduct screenings for mental health, developmental needs, trauma history, and child trafficking; provide referrals as needed.
  • Provide individual, family, and group therapy, crisis intervention, and psychoeducation.
  • Maintain effective working relationships with families, colleagues, and community agencies.
  • Participate in case review meetings and present accurate client summaries.
  • Ensure compliance with documentation standards in agency, state, and federal databases.
  • Demonstrate cultural humility and trauma‑informed practices.
  • Requires frequent travel; occasional out‑of‑area and overnight travel, including airline travel.
