Highland Rivers Behavioral Health in Georgia is seeking a Case Manager to join the Intensive Case Management (ICM) team in a community-based program for adults with severe and persistent mental illness. The role focuses on developing an Individual Recovery Plan (IRP) and coordinating medical, behavioral, housing, and social supports to prevent out-of-home placement.

• Must have strong client-facing communication skills.
• Ability to coordinate care across multiple providers.
• Maintain confidentiality and HIPAA compliance.

Responsibilities

• Community based position.
• Provide transportation for the individuals.
• Must visit individual's homes.
• Assist individuals with service needs and develop IRP.
• Partner with the individual and CORE services provider in IRP development.
• Advocate with other agencies to obtain resources for individuals.
• Provide crisis support and ongoing assessment.
• Maintain accurate paperwork and meet policy requirements.
• Manage caseload and complete monthly reports.
• Attend treatment team meetings and ensure service plan adherence.
• Uphold privacy and confidentiality per agency policies and HIPAA.
