Job summary

Highland Rivers Behavioral Health seeks an Intensive Case Manager to join its Community-Based ICM team in Georgia. You will collaborate with individuals to develop an Individual Recovery Plan (IRP) and coordinate medical, behavioral, social, educational, vocational, housing, and other supports to reduce need for more intensive services.

You will link with CORE services providers, advocate with other agencies, and provide crisis intervention, assessment, and ongoing case management to maintain

Responsibilities

• Provide transportation for the individuals.
• Must visit individual's homes.
• Assist individuals with service needs, developing strategies and supportive networks.
• Develop the Individual Recovery Plan (IRP) with the individual and CORE services provider.
• Coordinate rehabilitation services as specified in the IRP.
• Link and advocate with other agencies to obtain resources.
• Provide assessment, monitoring, and crisis support.
