Job summary

The New York City Criminal Justice Agency (CJA) seeks a Clinical Case Manager II – Intensive Case Management in Queens. This role supports individuals with complex needs within a supervised release program, collaborating with clinicians, housing specialists, and partners to promote stability and court compliance.

Qualifications

• MSW or relevant master's degree required; two years of related case management experience.
• LMSW, LMHC, or LCAT licensure required.
• Familiarity with NYC Court Operations preferred.

Responsibilities

• Conduct comprehensive intake and needs assessments to identify mental health, substance use, housing, and other psychosocial needs.
• Provide trauma-informed, person-centered clinical case management to a high-risk caseload of youth and adults.
• Collaborate with a multidisciplinary team to develop and implement individualized service plans for stability and court compliance.
• Support participants in navigating behavioral health, housing, public benefits, and legal services.
• Maintain regular contact with participants through in-person and phone check-ins and monitor progress and court mandates.
