Qualifications

• MSW or relevant master\'s degree is required.
• LMSW/LMHC/LCAT licensure is required.
• Knowledge of NYC Court Operations is preferred.

Responsibilities

• Conduct comprehensive intake and needs assessments to identify mental health, substance use, housing, and other psychosocial needs.
• Provide trauma‑informed, person‑centered clinical case management to a high‑risk caseload.
• Collaborate with a multidisciplinary team to develop and implement individualized service plans.
• Support participants in navigating behavioral health, housing, benefits, and legal services.
• Maintain regular contact with participants and monitor progress and court compliance.
