You will provide community-based support to high-risk, high-need clients and work with a multidisciplinary team to promote recovery and access to services.

The role emphasizes trauma-informed, culturally sensitive care, collaboration with partners, and documentation of progress.

Qualifications

• Bachelor's Degree required with 2+ years in mental health, substance use, trauma, or related fields.
• Alternate: 5 years of relevant experience plus CASAC/CASAC-T.
• Fluency in Spanish is a plus.

Responsibilities

• Screen and assess clients for co-occurring substance use and mental health disorders using standardized tools and team input.
• Provide individualized interventions (CBT, crisis intervention, de-escalation) to manage crises.
• Develop and implement treatment plans to address behavioral issues.
• Advocate and liaise with criminal justice and shelter systems; support clients through treatment.
• Coordinate with hospitals, shelters, and primary care coordinators for engagement.
• Maintain accurate progress notes and ensure confidential recordkeeping.
