Qualifications

• Bachelor's degree in social work, psychology, counseling, or related field.
• Associate degree will be considered in combination with five years of experience.
• Minimum of two years of direct counseling experience; trauma-informed care knowledge beneficial.

Responsibilities

• Provide intensive, person-centered case management to program participants, focusing on housing stability and community integration.
• Conduct needs assessments and develop individualized care plans.
• Coordinate access to community-based services: housing, counseling, mental health, medical care, financial aid, employment support.
• Advocate for clients to ensure access to necessary services.
• Meet with clients regularly to provide support and crisis intervention, including evenings/weekends.
• Maintain client records and document participation in HMIS.
• Develop transition/discharge plans for permanent housing and ongoing support.
• Collaborate with staff and external providers to create recovery-focused care plans.
