Job summary

Catholic Charities of Tompkins/Tioga is seeking a Case Manager/Housing Counselor to support housing stabilization for ESSHI Permanent Supportive Housing participants. Under supervision, you will perform assessments, plan services, coordinate resources, and advocate for residents to enhance self-sufficiency.

Responsibilities include intake, service planning, and regular progress reviews, aligning with OASAS ESSHI guidelines and agency standards.

Qualifications

• Education: Associate degree in Human Services, Social Work, Psychology, Counseling, Sociology, Criminal Justice, or related field preferred.
• Experience: Minimum one year working with people experiencing homelessness, substance use disorders, domestic violence, mental health conditions, or other vulnerable populations.

Responsibilities

• Conduct housing pre-screenings and eligibility reviews.
• Complete intake assessments upon admission.
• Develop individualized Service Plans within 30 days of program entry.
• Coordinate benefits and public assistance applications (SNAP, Medicaid, etc.).
• Maintain resident charts and service documentation; ensure compliance with agency standards.
