Job summary
Catholic Charities of Tompkins/Tioga is seeking a dedicated Case Manager/Housing Counselor to provide housing stabilization and supportive case management under the ESSHI Permanent Supportive Housing Program. You will assess needs, develop service plans, coordinate resources, and advocate for residents in a trauma-informed, recovery-oriented framework.
Under supervision, you will conduct pre-screenings, admissions, and ongoing plan reviews while helping participants access public benefits,
Qualifications
- Associate degree in Human Services or related field preferred.
- At least 1 year of experience with homelessness or other vulnerable populations.
- Experience with ESSHI, OASAS guidelines is a plus.
Responsibilities
- Conduct housing pre-screenings and eligibility reviews.
- Complete intake assessments upon admission.
- Identify participant strengths, needs, barriers, supports, and service priorities.
- Develop individualized Service Plans within 30 days of program admission.
- Establish measurable goals related to housing stability, wellness, and self-sufficiency.
- Coordinate with health, behavioral health, employment, and education services.
