Job summary

Alcott, a nonprofit agency serving low-income adults in Los Angeles County, is seeking an Interim Housing Case Manager to join the Interim Housing team in Culver City, CA. You will manage a caseload, conduct weekly check-ins, and help clients access services and housing opportunities in a trauma-informed, harm-reduction framework.

You will collaborate with a multidisciplinary team, document progress, and support planning toward stability and permanent housing in a fast-paced, on-site setting.

Qualifications

• At least one year of experience working with individuals experiencing homelessness and a bachelor’s degree in social work, psychology, or a related behavioral health field
• OR two years of relevant experience and a high school diploma or GED
• Experience supporting clients with co occurring mental health, substance use, or chronic health conditions is strongly preferred
• Familiarity with housing navigation, crisis response, and public benefits systems

Responsibilities

• Meet with new clients within 24 hours of program entry to complete intake, provide a tour, and explain program rules and services
• Build rapport using trauma informed and harm reduction approaches to create a safe, supportive environment
• Complete intake documentation within required timeframes and coordinate room assignments with site staff
• Confirm medication intake with medical team and support continuity of care
• Conduct weekly check ins with assigned clients and document progress in CHAMP
• Support clients in obtaining critical documents such as IDs, benefits paperwork, and birth certificates
• Monitor client needs including mental health symptoms, substance use, and medication compliance
• Schedule and coordinate transportation for medical and housing related appointments
• Investigate concerns raised by staff and follow up with clients to address behavioral or engagement issues
• Collaborate with team members and attend multidisciplinary meetings to ensure coordinated care
• Develop and update individualized care plans aligned with client goals and needs
• Complete required assessments, maintain accurate records, and ensure compliance with agency and funder requirements
• Provide weekly progress updates to the Program Manager and contribute to service planning
• Perform additional duties as needed to support program success
