Qualifications

• One year Case Management experience preferred.
• Two years' general work experience providing support to individuals in behavioral health, Addiction Recovery or IDD services required.
• Valid driver’s license.

Responsibilities

• Determine resources needed to meet the individual needs of participants.
• Provide community-based services focusing on education and achieving recovery plan goals.
• Complete all assessments and documentation as required.
