Job summary

Middle Flint Health & Wellness is seeking a Case Manager to join the Intensive Case Management Team in Georgia. The role focuses on delivering community rehabilitative services, direct treatment, and robust case management to reduce symptoms and promote independent living.

Responsibilities include maintaining a 15–30 client caseload, documenting visits in the electronic system within 24–48 hours, and ensuring timely connections to housing and employment resources.

Qualifications

• Bachelor’s Degree in psychology or related field required.
• Must have a valid, unrestricted Georgia Driver’s License.
• Keyboard proficient.

Responsibilities

• Manage a caseload of 15-30 individuals with regular home or community visits.
• Record visits and contacts in the electronic client database within 24–48 hours.
• Maintain productivity of 55% or higher each month.
• Provide follow-up and referrals for housing and employment as needed.
• Transporting clients is a required duty.
