Job summary

Middle Flint Health & Wellness in Georgia seeks a full-time Case Manager for the Intensive Case Management Team. You will deliver community rehabilitative services, provide direct treatment and coordinate care to support clients in reducing symptoms and achieving independent living and interpersonal skills.

Manage a caseload of 15–30 individuals with at least 4 face-to-face contacts per month, document visits within 24–48 hours, and maintain a productivity of 55% or higher.

Qualifications

• Bachelor’s Degree in psychology or related field.
• Valid, unrestricted Georgia Driver’s License.
• Keyboard proficient.

Responsibilities

• Manage a caseload of 15-30 individuals with home or community visits.
• Document visits and interactions in the electronic client database within 24-48 hours.
• Meet productivity target of 55% or higher monthly.
• Coordinate with physicians and service providers for continuity of care.
• Provide on-call support and transportation as needed.
