Job summary

Job description

Our mission is to continuously improve the health and independence of the persons we serve.

Responsibilities

Under the supervision of the Clinical Program Manager, the case manager provides recovery-oriented case management services, skill building, and medication education.

The case manager conducts routine psycho-social-emotional needs assessments, completes Person-Centered Recovery Plans, assists youth, adults, and families with accessing community resources, provides rehabilitative services, and initiates crisis intervention when needed.

All services are documented in an electronic health record and delivered in collaboration with a multidisciplinary recovery team. A high level of detailed electronic documentation is required, and adherence to company and state guidelines is expected. Minimum 55% of hours worked must be direct service.

Qualifications

• Bachelor’s degree in Human Services (or equivalent) – 24 credits in a human services field if degree is not in Human Services
• Comprehensive knowledge of mental health diagnoses and treatments
• Ability to work independently and collaboratively in a fast-paced, recovery-focused environment
• Demonstrated aptitude to connect with people
• Valid Texas Driver’s License with approved driver status throughout employment

Preferred Qualifications

• Master’s degree in Human Services
• One year of experience providing wellness and recovery support services in mental health, substance use, and/or crisis intervention and screening
• Pursuit of clinical licensure in counseling, behavior analysis, family therapy, social work, or a related field
• Advanced licensure
• Bilingual in ASL or Spanish
• Experience in concurrent documentation
• Licensed Chemical Dependency Counselor (LCDC) licensure

Benefits

Hourly Salary Range: $21.46 - $28.71

• Up to $4.00/hour - Tenure with BTCS
• $0.75/hour - Bilingual
• $1.00/hour - Prior experience in a similar role
• $1.00/hour - Associate or Advanced Licensure
• $0.50/hour - LCDC Licensure
