Furthering our Mission to continuously improve the health and independence of the persons we serve, the case manager under the supervision of the Clinical Program Manager provides recovery-oriented case management services, skill building, and medication education. The case manager conducts routine psycho-social-emotional needs assessments, completes Person- Centered Recovery Plans, assists youth, adults, and their families with accessing community resources, provides rehabilitative services, and initiates crisis intervention when needed. All services are documented in an electronic health record and provided in collaboration with a multidisciplinary recovery team.
Services promoted include recovery and wellness, community integration, independent living within the community, and improved quality of life. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected. A minimum of 55% of hours worked must be direct service.
Required skills and knowledge include passion for working with individuals who have chronic and severe mental health needs; knowledge of psychotherapeutic medications, case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the behavioral health field. Effective case management employs innovative thinking, patience, resiliency, advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning, clinical documentation skills, and dual-diagnosis treatment. The case manager works independently and within a team, prioritizes needs during crisis situations, engages in regular meetings and training opportunities, and performs other duties as assigned with a positive attitude.
