Job summary

Ada S McKinley Community Services Inc. seeks a Care Coordination and Support: Intensive Care Coordinator (CCSI) to drive wraparound services for youth with mental health needs.

You will lead family team meetings, coordinate with professionals, and help children meet goals in home, school, and community. This role involves trauma-informed outreach, intensive care coordination for about 16–18 families, strong documentation, and collaboration with Ada S McKinley’s MCR team.

Qualifications

• Experience working with children/adolescents and families is required.
• Experience with care coordination is a plus.
• Excellent communication, organization, and pc/computer skills (Outlook, Word, Excel and PowerPoint).
• Bilingual is preferred.

Responsibilities

• Perform outreach and engagement to locate, educate Pathways youth and their families.
• Use trauma-informed approach to engage youth with behavioral health needs and families.
• Provide intensive care coordination with safety planning and wraparound development.
• Schedule, plan and facilitate Child & Family Team Meetings.
• Builds knowledge of community resources and links to supports.
• Provide regular communication with community partners and coordinate services.
• Coordinate services with community resources, placement providers, and other agencies.
• Facilitate the creation of safety and crisis plans.
• Collaborate with Ada S. McKinley MCR team when necessary.
• Enact Ada S. McKinley Care Coordination Model for individuals and families.
• Facilitate application processes and consents for SFSP/FSP eligibility.
• Provide care coordination to SFSP/FSP eligible youth and families.
• Complete service documentation aligned with standards.
• Helping find services in the community, strong writing and organizational skills, self-starter and multitasker.
