You will lead care coordination for youth with behavioral health needs, organizing team meetings, coordinating services, and documenting progress to support home, school, and community functioning. The role emphasizes engagement with families, trauma-informed practices, and collaboration with multiple partners to ensure wraparound services and effective

Qualifications

• Bachelor’s degree in social work, counseling, psychology or related field required.
• Two years of supervised clinical experience in a mental health setting required.
• Experience working with children/adolescents and families is required.
• Bilingual is preferred.

Responsibilities

• Perform outreach and engagement to locate, educate Pathways youth and their families.
• Coordinate with professionals and facilitate Child & Family Team Meetings.
• Provide intensive care coordination using a strengths-based approach to safety and wraparound planning.
• Maintain knowledge of community resources and link youth and families to supports.
• Collaborate with local agencies and Ada S. McKinley teams as needed.
• Ensure documentation aligns with agency standards.
