Job description

Duties and Responsibilities:

The primary role of our Intensive Care Coordinator (ICC) is to facilitate implementation of the cross-system/ multi-agency collaborative care by working collectively with all members of the youth’s treatment team to support the youth’s success throughout their course of treatment. The ICC facilitates the provision of comprehensive services in a coordinated, community-based, developmentally appropriate, and trauma-informed manner. The ICC utilizes an all-inclusive approach and promotes service access, delivery, and transition. All services provided by the ICC are trauma-informed, culturally sensitive, strength-based, needs-driven, and family-focused. Services provided by the ICC align with the mental health Recovery Model and Core Practice Model (CPM) and support positive youth and family outcomes. A core function of the ICC is to foster strong engagement and participation with the youth, their family, community partners, and formal and informal support systems. The ICC focuses on the needs and strengths of the youth and their family and is responsible for the coordination of services for all youth on their caseload. This includes, but is not limited to, the facilitation of Child and Family Team Meetings (CFTs), planning, tracking progress, coordinating and providing aftercare support, linking youth and families to resources and supports, initiating referrals, providing Targeted Case Management, providing collateral services, collaborating with providers, services, and agencies such as County Mental Health, Placement Agencies, Social Services, Probation Department, Police Department, and tribal partners (when applicable). All services facilitated and provided by the ICC are done in a culturally sensitive manner that emphasizes respect for diversity, and collaboration.

Minimum Qualifications:
  • Bachelor’s or Master’s Degree in Social Work or other mental health discipline required
  • Experience navigating child/family-serving systems and working collaboratively with state agencies, consumer advocacy groups, and/or behavioral health facilities
  • Experience in working with and advocating for children/families with serious emotional/behavioral health needs
  • Knowledge of Wraparound systems of care and child welfare
  • Excellent oral, written, organizational and time management skills
  • Willingness to work flexible hours including evening and some weekend hours
  • A valid driver’s license and reliable automobile, as travel is required
  • Basic computer fluency, including Microsoft Office

Part Time Position.
