• Experience working with Children/adolescents and families is required.
• Bachelor’s degree in a related human service field or two years of supervised clinical experience in mental health.

Responsibilities

• Perform outreach and engagement to locate, engage, and educate Pathways youth and families.
• Provide intensive care coordination and wraparound planning to maintain children in home, school, and community.
• Schedule, plan and facilitate Child & Family Team Meetings.
• Link youth and families to community resources and supports.
• Coordinate services from community resources, placement providers, and other partners.
• Complete service documentation in alignment with standards.
