Job summary

Kenneth Young Center in the Chicago area is hiring a Care Coordinator for the Pathways to Success program. You will engage eligible participants and families in home and community-based services, assess needs, plan, and monitor service delivery.

This role emphasizes family-driven, strengths-based care, with caseloads of youth under 21 who are Medicaid/MCO enrolled. You will provide in-person, video, and telephonic outreach and coordinate with teams and post-discharge planning.

Qualifications

• Bachelor's or Master's Degree in Social Work, Psychology, Counseling.
• Minimum of 2 years experience providing clinical treatment or care coordination with children and adolescents.
• Valid driver's license, proof of insurance and reliable transportation.
• Proficient with Microsoft Word & Excel.

Responsibilities

• Provide Integrated Assessment and Treatment Plan reviews (IM+CANS) for community-based clients.
• Offer stabilization and support services to clients and families with a strengths-based approach.
• Coordinate care with MCOs, providers, and child-serving systems.
• Lead and participate in Child and Family Team meetings.
• Create crisis prevention plans and monitor progress.
• Engage in outreach in-person, via video, and by telecommunication.
• Conduct regular clinical supervision and participate in trainings.
