Job summary

Kenneth Young Center seeks a dedicated Care Coordinator for the Pathways to Success program in the Chicago area. The role engages eligible youth and families in home- and community-based services, assessing, planning, and monitoring service needs with a strengths-based approach.

You will work to connect clients to long-term supports and community resources. Responsibilities include conducting integrated assessments, coordinating care with MCOs, and leading team meetings while maintaining core

Qualifications

• Bachelor’s or Master’s Degree in Social Work, Psychology, Counseling or closely related discipline.
• Minimum of 2 years experience providing clinical treatment or care coordination with children and adolescents.
• Valid driver’s license, proof of insurance and reliable transportation.
• Computer skills including Microsoft Word & Excel.

Responsibilities

• Provide Integrated Assessment and Treatment Plan updates (IM+CANS) for community-based clients.
• Provide stabilization and support services to clients and families.
• Coordinate care with MCOs, providers, other child-serving systems and supports.
• Coordinate Child and Family Team (CFT) meetings and document outcomes.
• Maintain CCS requirements with prescribed outreach, CPSP, and reporting timelines.
• Participate in staff trainings and clinical supervision.
