Job summary

San Diego Center for Children is hiring for a Wraparound/Clinical role in San Diego, CA. You will lead assessment, planning, and intensive case management for referred children and families, coordinating with various systems to ensure HOPE-informed care and strong family partnerships.

The position emphasizes high fidelity wraparound, crisis intervention, and cross-agency collaboration, with opportunities for professional development and advancement.

Qualifications

• Master’s Degree from an accredited university.
• Valid California Driver’s License and reliable transportation.
• Registered Associate or in process with the BBS and license eligible in California.
• Excellent written and verbal communication skills.
• Experience working with high-risk youth.
• Ability to work cohesively on a team.

Responsibilities

• Provides assessment, evaluation, plan development, collateral, crisis intervention, and intensive case management services.
• Facilitates the High Fidelity Wraparound Team development and process, ensuring smooth transition through the four phases of wraparound and inclusion of natural supports.
• Develops, coordinates, and ensures the implementation of the client plan and wraparound plan of care.
• Monitors data on treatment progress and uses it to modify the plan as needed.
• Locates, coordinates and develops linguistically and culturally diverse resources for families in their communities.
• Oversees crisis plans and provides 24/7 on-call crisis intervention when needed.
• Collaborates with CFWB Social Workers, Probation Officers, and outside resources such as schools and community programs.
• Participates in high fidelity and quality improvement efforts including wrap coaching and supervision.
• Maintains compliance with Medi-Cal requirements and organizational policies.
