Easterseals MORC is seeking an Intensive Care Coordinator - Wraparound to support families and youth in a collaborative, community-based setting. The role emphasizes fidelity to the wraparound model and ongoing family collaboration.

Applicants should hold CMHP eligibility, a related bachelor’s degree, and a valid Michigan driver’s license. The position offers a hybrid work arrangement and opportunities for professional growth with benefits and supportive programs.

Qualifications

Responsibilities

• Engage individuals in a welcoming, hopeful, empathic manner.
• Convene the Child and Family Team and guide wraparound process.
• Maintain a caseload of 8–12 families with weekly face-to-face contact.
• Facilitate wraparound meetings within two weeks of enrollment.
• Develop Strength Assessments and Needs and Priorities within one week.
• Create an individualized plan of care with community-based services within 45 days and update every 90 days.

Qualifications:

• Possess a Bachelor’s degree from an accredited college or university with a major in Social Work, Psychology, or a related Human Services field.
• Must qualify as a Children's Mental Health Professional (CMHP) in accordance with the Michigan Medicaid Provider Manual. CMHP qualification requires meeting the position's educational requirements AND one of the following criteria:
• Three years of supervised experience in the examination, evaluation, and treatment of minors and their families; OR
• Licensed as an LLBSW, LBSW, LLMSW, LMSW, LLPC, LPC, TLLP, or LLP in the State of Michigan AND one year of experience in the examination, evaluation, and treatment of minors and their families.
• Must possess and maintain a valid Michigan driver's license and current auto insurance.

Duties and Responsibilities:

• Demonstrate the ability to engage individuals in a welcoming, hopeful, empathic manner regardless of disability or phase of recovery.
• Convene the Child and Family Team and guide members through the wraparound process, ensuring adherence to model fidelity and wraparound values.
• Maintain a caseload of 8 to 12 families with a minimum of one face-to-face contact per week.
• Facilitate a wraparound team meeting within two weeks of enrollment by interviewing the family and identifying family members, natural supports, agency representatives, and other significant persons.
• Work with the family to identify strengths and needs, address immediate needs, and complete the initial Strength Assessment and Needs and Priorities within one week of enrollment.
• Work with the family and submit a comprehensive Support Plan, including proactive and reactive crisis and safety plans, within one week of enrollment. Ensure the plan reflects the best possible fit with the family’s strengths, culture, and beliefs.
• Work with the Child and Family Team and submit an Individualized Plan of Care with community-based and culturally relevant services and resources within 45 days of enrollment, then update the plan every 90 days
